Required Forms
Complete all forms in their entirety for participation in the competitive sport program which includes, 6th - 8th grade teams, Junior Varsity sports and Varsity sports programs.
All student-athletes must register using the online AthleticClearance.com process.
Downloadable Forms / Information Sheets
This form must be downloaded and completed by a physician. Once complete, you will need to upload the completed document into the AthleticClearance.com portal.
